Komen Navigation Program Keeps Patients in Continuum of Care
One of the most important priorities of the Komen Charlotte Navigation program is to keep breast cancer patients in the continuum of care. Follow through on treatment plans, medications, and scheduled appointments is critical to a successful cancer journey. However, sometimes life happens and prevents people from getting the care they need. Luckily, we were able to help Stella*, a 56-year old woman and breast cancer survivor, get back on track when her circumstances changed and she ran out of resources and an ability to follow through.
When Stella called our office, she was immediately connected with Care Ring, a partner whose Breast Health Initiative is funded in part by a Komen Charlotte grant. Stella was first diagnosed with breast cancer in 2013, and received treatment and care until 2015. Soon after, she lost her health insurance and was unable to pay for her medication and follow-up visits with her doctors. This long gap in follow-up care, combined with the financial hardship of seeking medical intervention, made it difficult for Stella to resume the care she needed.
Thankfully, through Komen navigation and funding, partners at Care Ring were able to enroll Stella in Physician Reach Out, a local charity care program. She was assigned to an oncology team to receive critical diagnostic and treatment services. She also received breast health education and emotional support and counseling. We are glad that Stella is back on track with a care team, is currently cancer-free, and attends regular appointments.
Every day, we receive requests from survivors and MBC Thrivers who need resources and financial assistance, and women who are experiencing symptoms and do not know where to turn. Through our Navigation and Resource Fund and your generosity, we will continue to help even more people in our community.
